INTRODUCTION

What is the definition of an abdominoplasty?

An abdominoplasty is aesthetic surgery on the stomach. The following terms are sometimes used to refer to an abdominoplasty : tummy tuck, aesthetic abdominal surgery or tummy lift.

What is the purpose of an abdominoplasty?

  • An abdominoplasty removes the most damaged skin from the stomach (removing stretch marks, an abdominal apron or scars) and tightens the healthy skin by means of a tummy lift. If all the skin of the stomach needs tightening, the best abdominoplasty technique is a superior high tension abdominoplasty . If only the skin of the stomach below the navel needs tightening, a mini-abdominoplasty is an option.
  • An abdominoplasty can treat an excess of localised fat using lipoaspiration (from the epigastrium or abdominal flanks). Lipoaspiration from the whole surface of the stomach, to reduce amounts of stomach fat, is dangerous if done at the same time as a high tension abdominoplasty and is never performed.
  • An abdominoplasty can correct muscle wall abnormalities (diastasis and hernia)

SUPERIOR HIGH TENSION ABDOMINOPLASTY

A major advance in aesthetic abdominal surgery has been the development of the superior high tension abdominoplasty, which boasts many advantages over other techniques.

Advantages of superior high tension abdominoplasty

  • Tightening of the stomach is more intense and longer lasting
  • Complications such as haematoma, seroma or necrosis have become very rare

Purpose of superior high tension abdominoplasty

  • A high tension abdominoplasty restores the skin of the whole surface of the stomach, both above and below the navel, unlike a mini abdominoplasty

Technical principles of superior high tension abdominoplasty

  • A preliminary lipoaspiration is performed on the abdominal flanks, iliac fossa and epigastrium. Further information: liposuction
  • If necessary, diastasis of the rectus abdominis muscle is treated using sutures to approximate the muscles
  • All skin of the stomach below the navel is removed. Stretch marks are usually found in this area
  • The skin of the stomach above the navel is gradually tightened using internal sutures that progressively draw the skin down to the pubis
  • All internal detachment is closed with sutures; there is no longer any dead space with this technique (less risk of subsequent hematoma or seroma)
  • There are two scars produced by a high tension abdominoplasty: a horizontal scar at the lower edge of the stomach that is hidden in the underwear or bikini line (a Caesarean scar extended on both sides) and a small scar around the navel

Sometimes, mainly after weight loss, an abdominoplasty is combined with a lateral (outer) thigh lift and a buttock lift in a single operation called a body lift. Further information: body lift

MINI-ABDOMINOPLASTY

Purpose of a mini-abdominoplasty

A mini-abdominoplasty tightens the skin of the stomach below the navel.

Advantages of a mini-abdominoplasty

  • It results in only one horizontal scar in the bikini line area, with no scar around the navel
  • Liposuction of the whole surface of the stomach, to reduce the amount of abdominal fat, can be performed at the same time
  • Recovery is shorter than with a high tension abdominoplasty

Technical principles of a mini-abdominoplasty

  • Preliminary lipoaspiration is performed on the whole stomach
  • Part of the abdominal skin below the navel is removed and the remaining skin of the stomach below the navel is pulled towards the pubis.
  • With a mini-abdominoplasty, there is only one horizontal scar at the lower edge of the stomach that is hidden in the bikini line (a Caesarean scar extended on both sides), and no scar around the navel

INVERTED ABDOMINOPLASTY

Purpose

An inverted abdominoplasty tightens the skin of the upper stomach located between the breasts and the navel. It is recommended when the upper part of the stomach above the navel requires attention. It can be performed at the same time as a mastopexy or breast reduction.

Advantages of an inverted abdominoplasty

  • It produces only one horizontal scar in the inframammary fold (the scars meet between the breasts), with no scar around the naval
  • Liposuction of the stomach, to reduce the amount of abdominal fat, can be performed at the same time

Technical principles of an inverted abdominoplasty

  • Preliminary lipoaspiration is performed on the whole stomach
  • Part of the abdominal skin below the breasts is removed and the remaining skin of the stomach is pulled upwards.
